Emergency General Surgery – 24/7

Mr. Nicholas Iyamitai leads emergency surgical care for life-threatening conditions:

  • Acute appendicitis: Severe right lower abdominal pain, fever, vomiting. We perform appendicectomy (removal of appendix) usually within hours of diagnosis.
  • Strangulated hernia: A lump in the groin or abdomen that becomes painful, firm, and cannot be pushed back. Emergency surgery to prevent bowel death.
  • Bowel obstruction: Severe abdominal pain, vomiting, constipation, distended abdomen. We identify the cause (adhesions, hernia, volvulus) and operate to relieve the blockage.
  • Perforated viscus: Hole in stomach, small bowel, or large bowel causing peritonitis (severe abdominal pain, rigid abdomen, fever). Emergency laparotomy to close the perforation and wash the abdomen.
  • Acute cholecystitis: Gallbladder inflammation. We remove the gallbladder (cholecystectomy) when indicated.
  • Trauma laparotomy: For internal bleeding or organ injury after road traffic accidents, falls, or stabbings. We explore the abdomen, control bleeding, and repair damaged organs.
  • Abscess drainage: Large pus collections from appendicitis, diverticulitis, or post-operative infections. We drain under local or general anaesthesia.

Elective General Surgery

  • Umbilical hernia repair: Common in women after pregnancy. We repair with or without mesh.

  • Lipoma and sebaceous cyst removal: Benign lumps under the skin. We remove them in clinic under local anaesthesia.
  • Circumcision: For medical reasons (phimosis, recurrent balanitis) or cultural preference. We perform sterile circumcision with local anaesthesia.

In collaboration with Michael Emusugut (Orthopaedics), we manage:

  • Bite wounds (dog, human, animal): Cleaning, debridement, rabies prophylaxis, and antibiotics.
  • Burn management: Debridement of dead tissue, dressing, and grafting for deep burns (in collaboration with plastic surgery referrals when needed).

  • Nail bed injuries and finger tip amputations: Repair and reconstruction.

Our Obstetrician/Gynaecologist performs:

  • Caesarean section: Emergency or planned.
  • Manual vacuum aspiration for incomplete miscarriage or abortion: Safe, same-day procedure.
  • Ectopic pregnancy surgery: Laparotomy or laparoscopic salpingectomy (fallopian tube removal).
  • Ovarian cystectomy: Removal of large or symptomatic ovarian cysts.

Pre-Operative Assessment at Michoes

Before any surgery (elective or emergency when possible), we perform:

  • Full history and physical examination by Mr. Nicholas Iyamitai
  • Blood tests: haemoglobin, white cell count, platelets, blood group, HIV, hepatitis B, blood glucose, kidney and liver function if indicated
  • Urinalysis
  • Chest X-ray and ECG for older adults or those with heart/lung disease
  • Informed consent explaining risks, benefits, alternatives, and post-operative care
  • Nil-by-mouth instructions (when safe) to prevent aspiration during anaesthesia

Surgical Procedures We Do NOT Perform (Referral Pathway)

For complex procedures beyond our current scope, we stabilise and refer to Soroti Regional Referral Hospital, Mbale Regional Referral Hospital, or Mulago National Referral Hospital:

  • Cardiothoracic surgery (heart, lungs)
  • Neurosurgery (brain, spine tumours or trauma)
  • Major vascular surgery (aortic aneurysms, complex bypass)
  • Complex hepatobiliary and pancreatic resections
  • Organ transplantation
  • Major plastic surgery and microsurgery
  • Laparoscopic cholecystectomy and advanced laparoscopy (we do open cholecystectomy only)
  • Major joint replacements (hip, knee – we refer orthopaedic surgeons in Kampala)

Post-Operative Care and Recovery

Do I need to travel to Kampala for hernia surgery?

No. Mr. Nicholas Iyamitai performs safe, effective hernia repair at Michoes for a fraction of the cost. Most patients go home within 2 days.

How long does recovery take after appendicectomy?

Open appendicectomy: 2-4 weeks before returning to heavy work or farming. Laparoscopic (if available elsewhere) is faster. We give clear return-to-activity instructions.

What anaesthesia do you use?

For minor procedures: local anaesthesia (you are awake, area numbed). For larger procedures: general anaesthesia with airway support and monitoring, administered by trained staff. We do not have a specialist anaesthetist on site every day but we coordinate with visiting anaesthetists for complex cases.

Can I stay overnight after surgery?

Yes. We have post-operative observation beds. Most major surgeries (appendicectomy, hernia repair, caesarean section) include 2-3 days of inpatient stay.

Do you do keyhole (laparoscopic) surgery?

Not routinely. We perform open surgery for most general surgical procedures. For conditions requiring laparoscopy, we refer to Mbale or Kampala.

What happens if I need blood transfusion during surgery?

We cross-match blood pre-operatively when needed. If transfusion is required, we arrange from regional blood banks. Family donation is also possible with proper screening.
