Men’s Health Guide – Kumi, Soroti & Eastern Uganda

Men in Eastern Uganda often ignore their health until it becomes an emergency. At Michoes Medical Centre in Kanyum, we provide comprehensive men’s health services: prostate screening, heart health, sexual health, and chronic disease management. Prostate Health: Screening for Prostate Cancer

Prostate cancer is the most common cancer in Ugandan men. Early prostate cancer has no symptoms. Screening saves lives. We recommend annual screening starting at age 50 (or age 40 if father or brother had prostate cancer). Dr. Nicholas Iyamitai offers: digital rectal exam (DRE – feeling the prostate) and PSA blood test (prostate-specific antigen). High PSA may indicate cancer, but also infection or enlargement. If screening is abnormal, we refer for prostate biopsy (ultrasound-guided). Treatment options include surgery (prostatectomy), radiation, hormone therapy, or watchful waiting. Prostate cancer is slow-growing. Many men live many years with good quality of life. Symptoms of advanced prostate cancer: difficulty urinating, blood in urine, bone pain (spine, hips).

Benign Prostatic Hyperplasia (BPH): Enlarged Prostate

BPH is non-cancerous prostate enlargement common after age 50. Symptoms: frequent urination (especially at night), weak urine stream, difficulty starting urination, dribbling at the end, feeling of incomplete bladder emptying, and sudden urge to urinate. BPH does not cause cancer but can severely affect quality of life. Dr. Nicholas Iyamitai offers: medications (tamsulosin to relax prostate muscles, finasteride to shrink prostate), minimally invasive procedures (Rezum, UroLift – steam or implants to open prostate), and surgery (TURP – transurethral resection of prostate). Untreated BPH can cause bladder stones, kidney damage, and inability to urinate (acute retention – emergency requiring catheter).

Erectile Dysfunction (ED) and Sexual Health

Erectile dysfunction is the inability to get or keep an erection firm enough for sex. ED is common (affects 50% of men over 40) and often treatable. Causes: diabetes, high blood pressure, heart disease, high cholesterol, obesity, smoking, alcohol, stress, anxiety, depression, low testosterone, and medication side effects. Dr. Michael Emusugut and our medical team evaluate with: physical exam, blood tests (blood sugar, cholesterol, testosterone), and sometimes ultrasound of penile blood flow. Treatment options: oral medications (sildenafil – Viagra, tadalafil – Cialis – we have generic, affordable versions), lifestyle changes (weight loss, exercise, stop smoking, reduce alcohol), testosterone replacement if low, vacuum erection devices, penile injections, and penile implants for severe cases. Low Testosterone (Hypogonadism)

Testosterone is the male hormone that drives muscle mass, bone density, libido, energy, and mood. Testosterone naturally declines with age, but some men have very low levels. Symptoms: low sex drive, erectile dysfunction, fatigue, decreased muscle mass, increased body fat (especially belly), depression, irritability, poor concentration, and loss of body hair. We diagnose with morning blood test (testosterone level). If low, we investigate cause (pituitary problem, testicular failure, obesity, medications). Heart Disease in Men

Heart disease is the leading cause of death in Ugandan men. Risk factors: smoking, high blood pressure, high cholesterol, diabetes, obesity, physical inactivity, family history, and stress. Warning signs of heart attack: chest pain or pressure (may radiate to arm, jaw, back), shortness of breath, nausea, cold sweat, and lightheadedness. Dr. Michael Emusugut and our emergency team provide: EKG (electrocardiogram), cardiac enzymes (blood test), oxygen, aspirin, nitroglycerin, and clot-busting drugs (thrombolytics). We refer to Soroti Regional Referral Hospital for intensive care. Prevention: stop smoking, control blood pressure (below 130/80), control diabetes, take cholesterol medication (statins) if prescribed, exercise 30 minutes daily, eat more vegetables and less fried foods, and reduce salt.

High Blood Pressure (Hypertension) in Men

Hypertension is a silent killer – no symptoms until it causes stroke, heart attack, or kidney failure. Normal blood pressure is below 120/80. Hypertension is 140/90 or higher. Dr. Okello John Bosco (Laboratory) measures your blood pressure. Treatment: lifestyle changes (low salt diet – no added salt, less processed food, exercise, weight loss, limit alcohol, stop smoking) and medications (amlodipine, lisinopril, losartan, hydrochlorothiazide). Most men need two or more medications. Check your blood pressure at home. Take medications daily even if you feel fine. Uncontrolled hypertension causes stroke (paralysis, inability to speak), heart attack, kidney failure (dialysis), and blindness.

Diabetes in Men

Men with diabetes have higher risk of heart disease, erectile dysfunction, kidney failure, and foot ulcers requiring amputation. Dr. Michael Emusugut sees many men with diabetic foot infections. Symptoms: frequent urination, excessive thirst, unexplained weight loss, blurry vision, slow-healing cuts, and numbness in feet. Our laboratory offers blood glucose and HbA1c testing. Treatment: metformin, other medications, insulin if needed, diet (reduce sugar, soda, white rice, white bread), exercise, and regular foot checks. Never walk barefoot. Inspect your feet daily. Sexually Transmitted Infections (STIs) in Men

Common STIs in men: gonorrhea and chlamydia (urethral discharge, painful urination), syphilis (painless sore on penis, then rash), genital herpes (painful blisters), genital warts (HPV – cauliflower-like bumps), and HIV. Untreated STIs cause infertility (blocked tubes), chronic pelvic pain, and spread to partners. Dr. Okello John Bosco performs urine tests, urethral swabs, and blood tests (syphilis, HIV, hepatitis B). Treatment: antibiotics (azithromycin, ceftriaxone, doxycycline) – take all doses. Partner treatment is essential to prevent reinfection. Use condoms to prevent STIs. HIV is manageable with daily antiretroviral therapy (ART). Lung Health: Smoking-Related Diseases

Smoking causes lung cancer, chronic bronchitis, emphysema (COPD), and heart disease. Symptoms: chronic cough (smoker’s cough), coughing up blood, shortness of breath, wheezing, and frequent chest infections. Alcohol and Liver Disease in Men

Heavy alcohol use causes alcoholic liver disease: fatty liver (reversible with abstinence), alcoholic hepatitis (inflammation – abdominal pain, jaundice, fever), and cirrhosis (scarring – incurable, leads to liver failure and death). Signs: yellow eyes and skin (jaundice), swollen abdomen (ascites – fluid), swollen legs, easy bruising and bleeding, confusion (hepatic encephalopathy). Treatment: complete abstinence from alcohol (no amount is safe), nutritional support (high protein, B vitamins), and referral for liver transplant (only in South Africa or India, very expensive). Prostate Infection (Prostatitis)

Prostatitis is infection or inflammation of the prostate. Symptoms: fever, chills, painful urination, pain in lower back, perineum (area between scrotum and anus), or testicles, and frequent urination. Acute bacterial prostatitis is an emergency – can cause sepsis. Dr. Nicholas Iyamitai treats with antibiotics (4-6 weeks), pain medication, and alpha-blockers (tamsulosin) to relax the prostate. Chronic prostatitis (pain without infection) is harder to treat, requiring long-term pain management. If you have fever with pelvic pain, come immediately.

Testicular Pain and Swelling

Testicular torsion (twisted testicle) is a surgical emergency. Signs: sudden severe testicular pain, nausea, vomiting, and high-riding testicle. This cuts off blood flow to testicle. If not untwisted within 6 hours, testicle dies and must be removed. Dr. Nicholas Iyamaiti performs emergency surgery. Other causes: epididymitis (infection of the tube behind testicle – treated with antibiotics), hydrocele (fluid-filled sac – usually painless, surgery optional), varicocele (enlarged veins – may cause infertility, surgery if painful or affecting fertility), and testicular cancer (painless lump – come for ultrasound).

Kidney Stones in Men

Kidney stones cause severe colicky pain in the back or side, radiating to the groin. Other signs: blood in urine, nausea, vomiting, painful urination, and inability to find a comfortable position. Dr. Okello John Bosco diagnoses with urine test (blood, infection) and ultrasound (to see stones). Small stones pass with: drinking 3-4 litres of water daily, pain medication, and tamsulosin (relaxes ureter). Large stones (over 5mm) may need: shock wave lithotripsy (ESWL – breaks stone from outside), ureteroscopy (camera up ureter to remove stone), or surgery. If you cannot urinate, have fever with stone, or pain uncontrolled, come to emergency.

Workplace Injuries and Occupational Health

Men in Eastern Uganda work in farming, construction, boda boda transport, and factories. Common injuries: fractures, dislocations, deep cuts, head injuries, back pain from heavy lifting, and hearing loss from loud machinery. Dr. Michael Emusugut treats workplace injuries. Frequently Asked Questions About Men’s Health in Eastern Uganda

At what age should I start prostate screening?

Age 50 for average risk. Age 40 if you have a father or brother with prostate cancer.

Is erectile dysfunction normal as I age?

More common, but not normal. Most ED is treatable. Come for evaluation.

Do I need a doctor for a hernia?

Yes. Hernias (bulge in groin or scrotum) do not heal on their own. They can strangulate (cut off blood supply to intestine), causing emergency surgery. Come for evaluation.

How often should I check my blood pressure?

At least annually. More often if you have hypertension, diabetes, or family history.

Can I still have children after vasectomy?

Vasectomy is permanent. Reversal is possible but expensive and not always successful. Consider vasectomy only if you are certain you do not want more children.
