Laboratory and Diagnostic Services Guide – Kumi, Soroti & Eastern Uganda

At Michoes Medical Centre in Kanyum, Okello John Bosco (Senior Laboratory Technician) leads our on-site laboratory. We provide rapid, accurate diagnostic testing for patients from Kumi, Soroti, Bukedea, Ngora, Katakwi, Amuria, Kaberamaido, Pallisa, Tororo, Mbale, and Jinja. Our laboratory is open 24 hours a day, 7 days a week – including public holidays and overnight. Most results are available the same day. We follow strict quality assurance protocols from haematology, chemistry, microbiology, and parasitology. No need to travel to Soroti or Mbale for blood tests. We do them here.

Malaria Testing: Rapid Diagnostic Tests and Microscopy

Malaria is the most common test at Michoes. We offer two methods: Rapid Diagnostic Test (RDT) – finger prick, results in 15-30 minutes, detects malaria antigens, sensitivity 95% for Plasmodium falciparum (the deadly species). Microscopy – gold standard, requires blood smear and staining, examined under microscope by Okello John Bosco, results in 1-2 hours, detects species and parasite density (how many parasites). When to test: fever, chills, headache, body aches, vomiting, suspected severe malaria (confusion, seizures, difficulty breathing). We test all ages including newborns, pregnant women, and elderly. Do not treat malaria without a test. False positives waste medication. False negatives kill. Come to Michoes for accurate diagnosis. If your RDT is negative but symptoms persist, we repeat test after 24 hours or perform microscopy.

Blood Glucose and Diabetes Monitoring

We perform: Random blood glucose (RBG) – any time of day; Fasting blood glucose (FBG) – no food for 8 hours; and HbA1c – measures average blood sugar over 3 months, no fasting needed. HbA1c is essential for diabetes management. Normal: below 5.7%. Prediabetes: 5.7-6.4%. Diabetes: 6.5% or higher. For known diabetics, target HbA1c below 7%. We also perform glucose tolerance test (GTT) – for diagnosing gestational diabetes in pregnancy (Dr. Susan Atai orders this at 24-28 weeks). Testing is quick – finger prick for glucose, or venous blood draw for HbA1c (results in 2 hours). We also provide glucometers for home monitoring and teach you how to use them.

HIV Testing and Counseling

We offer confidential HIV testing using rapid tests (Determine, Unigold, Stat-Pak) following Uganda Ministry of Health algorithm. Blood from finger prick or oral swab. Results in 20 minutes. Testing is free during government campaigns or affordable fee otherwise. Pre-test and post-test counseling provided by trained counselors. If positive, we link you to care: CD4 count (measures immune system – blood test, results in 2 days), viral load (measures amount of virus – sent to central lab, results in 2 weeks), antiretroviral therapy (ART) initiation – free from government programs, and partner testing. We also offer PrEP (pre-exposure prophylaxis) for HIV-negative high-risk individuals – daily pill to prevent HIV. HIV is not a death sentence. Pregnancy Testing and Antenatal Labs

We perform urine pregnancy tests (hCG) – results in 5 minutes, detects pregnancy as early as 10 days after missed period. Blood pregnancy test (quantitative hCG) – more sensitive, detects earlier, measures exact level (useful for monitoring early pregnancy viability). For antenatal care, Dr. Susan Atai orders: blood type (A, B, O, AB) and Rh factor (positive or negative – Rh negative mothers need anti-D injection), hemoglobin (checks for anemia – normal 11-15 g/dL), syphilis (RPR/VDRL – treat with penicillin to prevent congenital syphilis), hepatitis B surface antigen (HBsAg – positive mothers need baby vaccinated at birth), HIV (as above), urinalysis (checks for protein – pre-eclampsia, sugar – gestational diabetes, infection), and blood glucose for gestational diabetes screening. Most antenatal lab results ready same day.

Typhoid Testing

Typhoid fever (caused by Salmonella typhi) is common in Eastern Uganda, especially during rainy season. Symptoms: prolonged fever (stepped pattern – increases daily), headache, abdominal pain, constipation or diarrhea, rose spots (pink rash on chest), and slow heart rate relative to fever (relative bradycardia). We offer: Widal test (agglutination – detects antibodies, results in 1 hour) – less accurate but rapid, and blood culture (gold standard, results in 3-5 days) – takes time but definitive. Treatment: antibiotics (azithromycin, ceftriaxone, ciprofloxacin) – do not treat without test because many causes of fever. Prevention: clean water (boil or treat), handwashing, and typhoid vaccine (available at Michoes – two types: injectable or oral).

Urinalysis and Urine Culture

Urinalysis (dipstick) tests for: protein (kidney disease, pre-eclampsia), glucose (diabetes), ketones (diabetic ketoacidosis), blood (kidney stones, infection, cancer), leukocytes and nitrites (urinary tract infection), bilirubin and urobilinogen (liver disease). Results in 5 minutes. If infection suspected, we send urine for culture and sensitivity (2-3 days) to identify which bacteria and which antibiotics kill it. Common UTIs: E. coli, Klebsiella, Proteus. We treat with antibiotics (nitrofurantoin, ciprofloxacin, ceftriaxone). For recurrent UTIs, we investigate with ultrasound and refer to Dr. Nicholas Iyamitai for possible structural problems.

Stool Testing: Parasites and Occult Blood

We test stool for: ova and parasites (hookworm, roundworm, whipworm, tapeworm, giardia, amoeba) – symptoms: diarrhea, abdominal pain, bloating, weight loss, anal itching (pinworms). Results in 1-2 hours. Also stool occult blood (hidden blood) – screening for colon polyps or cancer, requires 3 separate samples. And stool culture (for Salmonella, Shigella, Campylobacter – bacterial diarrhea) – 2-3 days. Deworming medications: albendazole, mebendazole (for worms), metronidazole (for giardia or amoeba). We recommend routine deworming for children every 6 months and adults annually.

Complete Blood Count (CBC) and Anemia Testing

CBC measures: hemoglobin (low = anemia), white blood cell count (high = infection, low = viral or bone marrow problem), platelet count (low = bleeding risk – dengue, malaria, leukemia), MCV (mean corpuscular volume – helps classify anemia type: microcytic = iron deficiency, macrocytic = B12/folate deficiency). Causes of anemia in Eastern Uganda: iron deficiency (poor diet, bleeding), malaria (destroys red blood cells), hookworm (intestinal blood loss), sickle cell disease, and chronic kidney disease. Treatment: iron supplements (ferrous sulfate), folic acid, B12 injections, deworming, and treating underlying cause. Severe anemia (hemoglobin below 7) may require blood transfusion – we have blood available or refer to Soroti Regional Referral Hospital.

Liver and Kidney Function Tests

Liver function tests (LFTs): ALT, AST, ALP, GGT, bilirubin, albumin, total protein – assess for hepatitis (viral, alcoholic), cirrhosis, liver cancer, or bile duct blockage. Kidney function tests (KFTs): creatinine, urea, eGFR (estimated glomerular filtration rate) – assess for dehydration, acute kidney injury, chronic kidney disease (from diabetes, hypertension, glomerulonephritis). Electrolytes: sodium, potassium, chloride, bicarbonate – imbalances cause confusion, weakness, heart arrhythmias. These tests require venous blood draw (fasting for LFTs preferred but not always necessary). Results in 2-4 hours. Used to monitor chronic disease patients (diabetes, hypertension, hepatitis) and before surgery.

Cholesterol and Lipid Profile

Lipid profile measures: total cholesterol, LDL (bad cholesterol – clogs arteries), HDL (good cholesterol – protects heart), triglycerides (fat in blood). Need fasting for 9-12 hours (no food or drink except water). Normal values: total cholesterol below 200 mg/dL, LDL below 100 mg/dL (or below 70 for high-risk patients), HDL above 40 mg/dL (above 50 for women), triglycerides below 150 mg/dL. High cholesterol increases heart attack and stroke risk. Treatment: lifestyle (diet: less fried food, red meat, sugar; more greens, beans, fish), exercise, weight loss, statins (atorvastatin, rosuvastatin). We prescribe statins and monitor LDL levels every 3-6 months until goal reached.

STI Testing: Syphilis, Gonorrhea, Chlamydia

We test for: syphilis (RPR/VDRL – blood test) – treat with penicillin G (Benzathine penicillin injection). Gonorrhea (urine PCR or urethral swab) – treat with ceftriaxone injection plus azithromycin. Chlamydia (urine PCR) – treat with doxycycline or azithromycin. Herpes simplex (HSV) – viral PCR from blister fluid or blood antibody test (IgG, IgM) – treat with acyclovir. Chancroid (Haemophilus ducreyi) – culture or PCR – treat with azithromycin or ceftriaxone. All STI testing is confidential. We treat both partners to prevent reinfection. Use condoms to prevent STIs. Blood Grouping and Crossmatching

We determine ABO blood group (A, B, AB, O) and Rh factor (positive or negative). Blood grouping needed for: pregnancy (Rh negative mothers need anti-D immune globulin), before surgery (in case transfusion needed), blood donation, and for medical records. Crossmatching – testing donor blood against recipient blood to ensure compatibility – performed before transfusion. We maintain a small blood bank for emergencies (group O negative – universal donor, limited supply). For major transfusions, we refer to Soroti Regional Referral Hospital blood bank. Ultrasound Services (On-Site and Referral)

We have on-site ultrasound performed by trained sonographers or Dr. Susan Atai (for obstetrics). Uses: pregnancy – dating, anomaly scan, growth scan, placental location, amniotic fluid; abdominal – liver, gallbladder (stones), kidneys (stones, hydronephrosis), spleen, pancreas; pelvic – uterus (fibroids), ovaries (cysts, masses), bladder; thyroid – nodules, goiter; scrotal – testicular torsion, tumor, hydrocele; vascular – deep vein thrombosis (DVT) – blood clots in legs. Ultrasound is painless, no radiation, safe in pregnancy. Results immediately. For complex cases (Doppler, 3D, cardiac), we refer to Soroti or Mbale. No need to travel far for basic pregnancy or abdominal ultrasound – we do it in Kanyum.

X-Ray Services (On-Site Digital X-Ray)

We have on-site digital X-ray (computed radiography) performed by trained radiographers. Uses: fractures (arm, leg, ribs, spine), dislocations, chest X-ray (pneumonia, tuberculosis, heart failure, lung cancer), abdominal X-ray (bowel obstruction, kidney stones), and extremity X-ray (arthritis, foreign body). Digital X-ray means less radiation, immediate images, can be shared electronically. Results within 30 minutes (or immediately if emergency). We also accept referrals from other clinics – bring your request form. We do not perform X-ray on pregnant women unless absolutely necessary – we use shielding. For complex imaging (CT, MRI), we refer to Soroti Regional Referral Hospital or Mbale.

Sickle Cell Disease Testing

Sickle cell disease is common in Eastern Uganda. We offer: sickle cell screening (sickling test – solubility test, results in 1 hour) – identifies presence of hemoglobin S, but cannot distinguish trait (AS) from disease (SS). For diagnosis: hemoglobin electrophoresis (separates hemoglobin types – HbA, HbS, HbF, etc.) – blood sample sent to central lab, results in 1-2 weeks. Newborn screening: heel prick blood sample collected on filter paper – early diagnosis prevents death from infection (pneumococcal) with prophylactic penicillin and vaccination. If your child has sickle cell disease, we provide: folic acid, hydroxyurea (reduces pain crises), vaccination, penicillin daily until age 5, malaria prevention, and pain management. Do I need an appointment for lab tests?

No. Walk in anytime. For fasting tests (glucose, cholesterol), come early morning before eating. For other tests, come when convenient.
