Emergency Conditions We Treat

Trauma and Accidents

Eastern Uganda roads are busy. Boda boda accidents, car crashes, falls, farming injuries, and assaults are common. We provide:

  • Primary and secondary trauma survey (ABCDE: Airway, Breathing, Circulation, Disability, Exposure)
  • Wound cleaning, debridement, and closure (suturing)
  • Fracture immobilisation (splinting, casting) and closed reduction – led by Michael Emusugut
  • Emergency surgery for internal bleeding or organ injury – Mr. Nicholas Iyamitai on call
  • Tetanus prophylaxis and antibiotics
  • Blood transfusion for severe blood loss (cross-matching on site, we coordinate with blood bank)
  • Head injury assessment (Glasgow Coma Scale) and observation – if severe, we refer for CT scan

Medical Emergencies

  • Severe malaria: IV artesunate, IV fluids, oxygen, and monitoring for seizures, anaemia, renal failure, or respiratory distress.
  • Severe pneumonia / difficulty breathing: Oxygen via nasal prongs or mask, nebulised salbutamol, IV antibiotics, and monitoring for hypoxia.
  • Heart attack (myocardial infarction): Aspirin, oxygen, pain relief (morphine), and risk assessment. We initiate treatment and refer urgently to a cardiac centre.
  • Stroke (sudden weakness, speech difficulty): Immediate assessment, blood pressure control, aspirin (if ischaemic), and referral for CT head.
  • Diabetic emergencies: Blood glucose testing. For severe hypoglycaemia (low sugar): IV dextrose or glucagon. For diabetic ketoacidosis: IV fluids, insulin, and electrolyte monitoring.
  • Seizures (epileptic convulsions): IV or rectal diazepam to stop the seizure. We check blood glucose, oxygen, and treat underlying cause (malaria, meningitis, head injury).
  • Severe dehydration from diarrhoea or vomiting: IV fluids, oral rehydration, and treatment of cause (antibiotics if bacterial).
  • Severe allergic reaction (anaphylaxis): Adrenaline (epinephrine) injection, antihistamines, steroids, oxygen, and IV fluids.

Obstetric and Gynaecological Emergencies

Dr. Susan Atai leads emergency obstetric care for:

  • Severe pre-eclampsia / eclampsia (seizures in pregnancy): Magnesium sulphate, blood pressure control, and emergency delivery (caesarean section if needed).
  • Postpartum haemorrhage (heavy bleeding after delivery): Uterine massage, oxytocin, misoprostol, tranexamic acid, and if severe, surgical exploration.
  • Ectopic pregnancy (ruptured): Emergency laparotomy to stop bleeding and remove the pregnancy.
  • Obstructed labour: Emergency caesarean section.
  • Ruptured uterus: Life-threatening. Immediate surgery, blood transfusion.
  • Miscarriage with heavy bleeding or infection: Manual vacuum aspiration, antibiotics, and IV fluids.

Paediatric Emergencies

Dr. Adoa Pious handles emergencies in children:

  • Severe malaria with convulsions or difficulty breathing – IV artesunate, oxygen, diazepam, monitoring.
  • Severe pneumonia – oxygen, nebulisation, IV antibiotics.
  • Severe dehydration – IV fluids, ORS, and treatment of underlying diarrhoea/vomiting.
  • Meningitis (fever, stiff neck, bulging fontanelle, lethargy) – IV antibiotics, supportive care.
  • Status epilepticus (prolonged seizure) – diazepam, airway management, glucose, treat underlying cause.
  • Newborn sepsis or jaundice – IV antibiotics, phototherapy if available, or refer for neonatal care.

What to Do in an Emergency Before You Arrive

  • Call us immediately: Dial +256701364362. Tell the operator what happened, the person’s condition, and your location. We can give first aid advice and prepare for your arrival.
  • Stay calm. Panic will not help. Take a deep breath.
  • Do not move the person unnecessarily. If there is a neck or back injury, keep them still. Wait for trained help or carefully immobilise the spine.
  • Stop bleeding: Apply firm pressure to any visible bleeding wound with a clean cloth. Do not apply a tourniquet unless trained and very severe arterial bleeding.
  • If unconscious: Check if they are breathing. If not breathing, start chest compressions (pump hard and fast in the centre of the chest, 100-120 per minute, 5-6 cm deep) and call again for emergency guidance.
  • If having a seizure: Do not put anything in their mouth. Move objects away from them. Place them on their side after the seizure stops. Time the seizure. Call us.
  • If pregnant and bleeding or seizing: Come immediately. Call ahead so we prepare Dr. Susan Atai and the operating theatre.
  • If a child has difficulty breathing or is not waking up: Come immediately. Do not wait. Carry the child to Michoes or call for transport advice.

Important: Do not give the person any food, drink, or oral medication until assessed by a doctor. Do not try to drive yourself if you are severely injured or having chest pain – ask someone else to drive or call for help.

Emergency Transport and Ambulance Advice

Michoes does not yet have our own ambulance, but we can help coordinate:

  • Private hire taxis or boda bodas: For stable patients who can sit, a taxi or boda boda may be appropriate. For unstable patients (unconscious, severe bleeding, breathing difficulty), you need a vehicle that allows lying flat.
  • Local ambulance services: We can call Kumi District ambulance or Soroti Regional Referral Hospital ambulance if available. Be aware that response times may vary.
  • Family vehicle: If you have a car, lay the patient flat in the back seat if possible. Drive carefully but quickly to Michoes. Call us on the way so we are ready.

  • Emergency caesarean section: 450,000 (includes surgery, 3-day stay, medications)

Yes. We have observation beds. For overnight monitoring, we will assess your condition hourly, provide medications, and communicate with our on-call doctor. We refer to higher level facilities if you need ICU.
