Early Pregnancy: Confirmation and First Trimester

If you miss your period or have a positive home test, come to Michoes for: urine pregnancy test, blood pregnancy test (hCG), ultrasound to confirm viability and due date, and blood type, HIV, syphilis, and hepatitis B screening. Dr. Susan Atai will discuss nutrition, medications to avoid, danger signs (bleeding, severe pain), and schedule your first antenatal visit between 8-12 weeks.

Antenatal Care (ANC): Monitoring Mother and Baby

We follow Uganda Ministry of Health guidelines: at least 8 ANC visits during pregnancy. At each visit, Dr. Susan Atai or our midwives check: blood pressure, urine for protein (pre-eclampsia), weight, baby’s growth (fundal height), baby’s heartbeat, and your questions/concerns. We also provide:

Ultrasound scans: Dating scan (8-12 weeks), anomaly scan (18-22 weeks), growth scan (32-34 weeks). Our on-site ultrasound machine is available daily.

Immunizations: Tetanus toxoid (TT) vaccine to protect baby from neonatal tetanus. Two doses, one month apart.

Intermittent Preventive Treatment for malaria (IPTp): Sulfadoxine-pyrimethamine tablets starting at 13 weeks, each visit at least one month apart.

Iron and folic acid supplements: Prevents anemia. Take daily.

HIV, syphilis, and hepatitis B retesting: At first visit and again at 28-32 weeks.

Danger Signs During Pregnancy: When to Come Immediately

Dr. Susan Atai emphasizes: come to Michoes emergency department immediately (day or night) if you have: vaginal bleeding, severe abdominal pain, severe headache with blurred vision, convulsions (seizures), difficulty breathing, fever with chills, reduced fetal movement (after 28 weeks), leaking fluid before term, or persistent vomiting unable to keep food down. These may indicate miscarriage, ectopic pregnancy, pre-eclampsia, eclampsia, severe anemia, malaria, or preterm labor.

Nutrition During Pregnancy: Eating Well in Eastern Uganda

Good nutrition saves lives. Eat a variety of local foods: fresh vegetables (greens, cabbage, nakati, dodo), fruits (oranges, mangoes, bananas, passion fruit), proteins (beans, groundnuts, fish, eggs, chicken, beef), whole grains (brown rice, maize meal, millet, sorghum), and dairy (milk, yogurt, cheese if available). Drink plenty of clean water. Avoid: raw meat or fish, unpasteurized dairy, undercooked eggs, excessive caffeine (more than 2 cups of tea or soda per day), alcohol, and street foods of uncertain hygiene.

Preparing for Birth: Birth Planning at Michoes

We encourage all mothers to make a birth plan. Discuss with Dr. Susan Atai: preferred birth attendant (our midwives or Dr. Atai), pain relief options (breathing, massage, walking, warm bath, pethidine injection, epidural if available), who will cut the cord (we delay clamping for 1-3 minutes), skin-to-skin contact immediately after birth, breastfeeding within the first hour, and what to bring to the hospital (clean cloths, baby clothes, blankets, sanitary pads, snacks).

We recommend packing your hospital bag by 36 weeks. Keep it ready at home. Our facility is open 24 hours, so come anytime you have regular contractions, water breaks, or bleeding.

Labor and Delivery: What to Expect

When you arrive at Michoes in labor, our midwives will: check your blood pressure, temperature, and pulse; check baby’s heartbeat; perform a vaginal exam to see how dilated you are; monitor contractions; and admit you to our maternity ward. We provide clean, private delivery rooms. Dr. Susan Atai is available for complicated deliveries, vacuum or forceps if needed, and cesarean sections. Our operating theatre is on-site and ready 24/7.

We practice respectful maternity care. You will never be shouted at, ignored, or denied pain relief. Your birth partner (husband, mother, sister) may stay with you during labor and delivery. We support mobility, position changes, and eating light snacks during early labor.

Cesarean Section (C-Section) Care

Dr. Nicholas Iyamitai, Head of Surgery, and Dr. Susan Atai perform cesarean sections when necessary: baby in breech or transverse position, placenta covering cervix (placenta previa), baby in distress, labor not progressing, or multiple pregnancies (twins, triplets). A C-section is major surgery. We use spinal or general anesthesia so you feel no pain. You will stay in our maternity ward for 2-3 days after surgery. You can still breastfeed. Recovery takes 4-6 weeks. We provide pain medication, wound care, and follow-up.

Postnatal Care: Mother and Baby After Delivery

After delivery, you and your baby stay at Michoes for at least 24 hours (longer if C-section or complications). We check: your bleeding (lochia), blood pressure, temperature, uterine contraction, wound if C-section, mood (for postpartum depression), breastfeeding latch and milk production, baby’s breathing, temperature, feeding, jaundice, and weight. We also administer: vitamin K injection for baby (prevents bleeding), eye ointment (prevents infection), first vaccines (BCG and polio at birth, or before discharge), and hepatitis B vaccine.

We recommend postnatal visits at 1 week, 6 weeks, and 6 months. At 6 weeks, we check your physical and emotional recovery, discuss family planning, and complete baby’s immunizations.

Breastfeeding Support at Michoes

Dr. Adoa Pious, our Pediatrician, strongly recommends exclusive breastfeeding for the first 6 months. Breast milk provides perfect nutrition, antibodies against infections, reduces diarrhea and pneumonia, and lowers baby’s risk of obesity and diabetes later. For mothers, breastfeeding reduces bleeding after delivery, helps the uterus shrink, lowers risk of breast and ovarian cancer, and burns calories. If you have trouble latching, sore nipples, low milk supply, or engorgement, our lactation counselors can help. We do not give formula unless medically necessary.

Family Planning After Pregnancy

You can get pregnant again as soon as 3 weeks after delivery. Dr. Susan Atai provides family planning counseling and methods: breastfeeding amenorrhea method (LAM) for first 6 months if exclusively breastfeeding, contraceptive pills (progestin-only safe while breastfeeding), contraceptive injection (Depo-Provera every 3 months), implant (up to 3-5 years), IUD (copper or hormonal, up to 5-10 years), condoms (male or female), and tubal ligation (permanent, after you complete your family). Discuss your options at your 6-week postnatal visit.

High-Risk Pregnancies and Complications

Some pregnancies need extra care. Dr. Susan Atai specializes in high-risk conditions: pre-eclampsia (high blood pressure and protein in urine), gestational diabetes (high blood sugar during pregnancy), placenta previa (placenta covers cervix), placenta accreta (placenta grows too deep), multiple gestations (twins, triplets), preterm labor (before 37 weeks), intrauterine growth restriction (baby not growing well), Rh incompatibility, and history of previous C-section or stillbirth. We provide extra ultrasounds, non-stress tests, blood pressure monitoring, blood sugar checks, and early delivery if needed. Newborn and Child Health

Dr. Adoa Pious provides complete newborn and child care: well-baby visits (1 week, 6 weeks, 10 weeks, 14 weeks, 6 months, 9 months, 1 year, 15 months, 18 months, 2 years, then annually), growth monitoring (weight, length, head circumference), development screening (milestones), immunizations (BCG, polio, pentavalent, rotavirus, pneumococcal, measles, yellow fever, vitamin A supplementation), treatment of common illnesses (malaria, pneumonia, diarrhea, malnutrition), and nutritional counseling (complementary feeding starting at 6 months). Maternal Mental Health
